All student-athletes must complete each of the following requirements listed below in order to compete for Waukesha West. Once all requirements have been received and verified by the Athletic Office, the student will be issued a BLUE CARD. If a student is able to practice, but NOT eligible for competition, a SILVER CARD will be issued. The card should then be presented to the coach on the first day of practice. No paperwork or fees should ever be given to a coach! The Athletic Office is open daily 7am-3pm.
A physical must be on file for all student-athletes. Most students complete the physical form during their freshmen and junior years, while completing the alternate year form sophomore and senior years. Please reference the table below for valid dates in regard to all physicals.

Student-athletes must have a minimum 2.0 GPA with no more than one F/I. When quarter and semester grades are available at a grade check point, the more favorable grades for the student will be used to determine eligibility. Incoming freshmen for the 2016-2017 school year will not have their grades officially checked until the end of 1st quarter.
Student-athletes must pay a $150 athletic fee in order to participate in athletics at any level. There is no sport limit, and no family cap. If the student-athlete does not make the team, or chooses to leave the team prior to the first competition date, a full refund may be requested.
Student-athletes should have no outstanding financial obligations owed to the school district at the time of registration.
All incoming freshmen, new/transfer students, as well as any student-athlete who did not participate in athletics at Waukesha West High School during the 2015-2016 school year must atttend the Freshmen/New Student Orientation Meeting and sign the Co-Curricular Code of Conduct and WIAA Eligibility forms.
This year's meeting will take place Tuesday, August 16th at 6:15pm in the West Auditorium. *Due to this meeting taking place after the start of many fall sport programs, this requirement will be waived until August 16, 2016.
Copies of both documents are shown below, and the signature forms for both are conveniently located on the physical/alternate year forms (above). It is not necessary to print and sign the WIAA Eligibility form.
Student-athletes and parent(s)/guardian(s) must complete the state-mandated Concussion Form once per school year.
Student-athletes and parent(s)/guardian(s) must complete the School District of Waukesha Spectator Code of Conduct once per school year.
Student-athletes and parent(s)/guardian(s) must complete a Health Information Form for every sport season.
